The ground it plays on
Hormones work in axes: a signal from the brain tells a gland to release another hormone, and so on down the chain, with brakes that shut the process off. Intervening at one point has consequences both upstream and downstream. That is why this is the group where professional assessment stops being a recommendation and becomes essential.

There is a distinction that almost always gets lost. A compound being studied for something is not the same as achieving it. What the human evidence says
It has indeed been given to humans, but almost always in acute physiology studies or as a diagnostic test of GH secretion, plus one phase 1 pharmacokinetic study in nine healthy volunteers. There are no efficacy trials with chronic administration.
The limitation that matters most
It is not selective: by acting on the ghrelin receptor it also stimulates ACTH, cortisol and prolactin and sharply increases appetite, and its effect on GH depends on endogenous hypothalamic GHRH, so it is blunted in people whose axis is impaired.

Not approved as a treatment. Banned in sport by WADA.
