The ground it plays on
Two very different things live under the same label here: serious research into mitochondria, cellular senescence and DNA repair, and a market of promises built on studies that were never replicated. Telling them apart is most of the work, and this is the area where it matters most to look at who signed each study.
What it is studied for
Advertised as a nootropic and nerve repair agent.
There is a distinction that almost always gets lost. A compound being studied for something is not the same as achieving it. And even where it works in a dish or in a mouse, that says little about whether it works in a person or whether the benefit is worth the risk. The section below therefore carries more weight than everything above it.
What the human evidence says
There is no published clinical evidence in humans. Every study that can be located is in rodents: sciatic nerve regeneration, cardiac gene expression and experimental cerebral ischaemia. No trial in patients.
The limitation that matters most
Besides coming from the same Russian circle without independent replication, there is an enormous gap between animal models of nerve injury and the way it is used on forums as a 'nootropic' by healthy people. The ischaemia studies are in rats and frequently test Cortagen alongside Cortexin, which makes it impossible to attribute the effect to the tetrapeptide on its own.

Cautions and context
No studies in patients. The available work usually tests it alongside another product.
Tingling, numbness or weakness that will not go away calls for a neurological workup before any peptide with no clinical data, since the underlying cause changes the treatment completely.
