The ground it plays on
In aesthetics the route of administration decides almost everything. A peptide that works in a fibroblast culture may not cross intact skin, and then the formulation matters more than the active ingredient. That is why two products listing the same ingredient can give completely different results.

The limitation that matters most
The studies are small, last only a few weeks, and rely largely on subjective endpoints, and penetration through the stratum corneum is the point still to be solved. There are still no multi-year follow-ups, and its effect is gentler than that of botulinum toxin.
Regulatory status
Cosmetic ingredient (INCI Acetyl Hexapeptide-8), not a drug: no agency has approved it for a medical indication. The Cosmetic Ingredient Review panel judged it safe in its 2020 assessment. It is not on the WADA 2026 Prohibited List.

It is a cosmetic with a gradual effect that depends on the formula and the vehicle. The serious risk appears with a route other than topical: infections with mycobacteria have been described after applying it with needles under the skin.
