Tirzepatide: what it is and what the evidence says

A synthetic 39-amino-acid peptide that activates both the GIP and GLP-1 receptors, the two gut hormones that regulate appetite, stomach emptying, and the insulin response to glucose. A fatty acid chain binds it to albumin and extends its action.

How to read this page. It is written so you can check it: every claim about evidence hangs from a reference with its PubMed identifier, and we state just as plainly what is known and what is not. You will not find dosages or protocols anywhere on this site.

The ground it plays on

Metabolism is not a single lever. Talk about metabolic compounds usually collapses three separate mechanisms into one word. Appetite regulation is one. How well cells respond to insulin is another, and largely independent of the first. Mobilizing stored fat is a third. A compound may act on one and leave the other two untouched, so knowing which one is the difference between understanding a molecule and reciting its name.

What it is studied for

Weight loss, glucose control, cardiometabolic health.

There is a distinction that almost always gets lost. A compound being studied for something is not the same as achieving it. And even where it works in a dish or in a mouse, that says little about whether it works in a person or whether the benefit is worth the risk. The section below therefore carries more weight than everything above it.

What the human evidence says

It is backed by large phase 3 trials. The SURMOUNT program recorded mean weight losses of up to 21% at 72 weeks, and the SURPASS program supported it in type 2 diabetes. It was superior to Semaglutide in two head-to-head comparisons (SURPASS-2 and SURMOUNT-5), and SURPASS-CVOT showed cardiovascular non-inferiority against dulaglutide.

The limitation that matters most

In obesity without diabetes there is not yet a completed cardiovascular events trial; SURMOUNT-MMO is still ongoing. Weight tends to return after stopping (SURMOUNT-4), and the long-term course of muscle mass and bone still needs to be better understood.

Regulatory status

Approved by the FDA as Mounjaro for type 2 diabetes (2022; from 10 years of age in 2025) and to reduce major cardiovascular events in adults with type 2 diabetes at high risk (August 2026), and as Zepbound for chronic weight management (2023) and obstructive sleep apnea in adults with obesity (2024). In the European Union, Mounjaro is authorized for type 2 diabetes and weight management. It is not prohibited by WADA, which has tracked it in its Monitoring Program since 2026.

Status can change and varies by country. Always verify the rules in force where you live before drawing conclusions.

Cautions and context

Digestive effects cluster at the start and at each step up, and weight loss can take muscle with it if protein and strength training fall short. It is contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2, and it calls for attention to the pancreas, gallbladder, and hydration.

It is a prescription medicine that is started and adjusted with a physician, with particular care if you are on insulin or sulfonylureas, or if there is pregnancy, a history of pancreatitis, or digestive disease.

This is not medical advice. Any decision about a compound in this encyclopedia belongs to a healthcare professional who knows your history, what you already take and your specific situation. If something you are using produces a new symptom, that is not something to observe — it is something to consult.

What this page cannot give you

A reference page can take you this far. You know what Tirzepatide is and where its research stands, which already puts you ahead of most readers. Four questions remain that no encyclopedia can answer for you, because the answers depend on your own situation:

That is what the course is for

PeptiXtreme is ${CIF.LECCIONES} lessons that hand out no numbers. Each one ends with your starting point, what to watch along the way, the specific questions you take to a professional and the signals that mean stop. It includes per-module exams, a Certificate of Completion with a verifiable certificate ID, a Progress Tracker to follow how you are doing, and a human advisor you can actually write to.

Frequently asked questions

What is Tirzepatide?

A synthetic 39-amino-acid peptide that activates both the GIP and GLP-1 receptors, the two gut hormones that regulate appetite, stomach emptying, and the insulin response to glucose. A fatty acid chain binds it to albumin and extends its action. It is studied within the area of metabolic.

Is there human evidence for Tirzepatide?

It is backed by large phase 3 trials. The SURMOUNT program recorded mean weight losses of up to 21% at 72 weeks, and the SURPASS program supported it in type 2 diabetes. It was superior to Semaglutide in two head-to-head comparisons (SURPASS-2 and SURMOUNT-5), and SURPASS-CVOT showed cardiovascular non-inferiority against dulaglutide.

Is it approved by any health agency?

Approved by the FDA as Mounjaro for type 2 diabetes (2022; from 10 years of age in 2025) and to reduce major cardiovascular events in adults with type 2 diabetes at high risk (August 2026), and as Zepbound for chronic weight management (2023) and obstructive sleep apnea in adults with obesity (2024). In the European Union, Mounjaro is authorized for type 2 diabetes and weight management. It is not prohibited by WADA, which has tracked it in its Monitoring Program since 2026.

What is the dose of Tirzepatide?

We do not publish dosages. This is not an oversight. No single figure works for everyone: the regimen depends on a person's medical history, on whatever medication they already take, and on the goal they are chasing, which is something a professional weighs with the case in front of them. What you can build is the ability to walk into that appointment knowing what to ask.

Where can I learn to evaluate this properly?

This page tells you what Tirzepatide is and what the evidence says. The PeptiXtreme course teaches the other half: what you would measure before considering anything, what you would watch, the specific questions you would take to a professional and when you would stop.
